  • Identity & contact: name, address, phone, email, date of birth.
  • Health information: medical history, current concerns, symptoms, treatment notes, measurements, and any imaging or clinical reports you share.
  • Insurance & billing: insurer, policy number, billing address (only when you choose to submit through us).
  • Appointment & communication records: session notes, emails, messages, and visit history.
  • Website usage: browser type, pages viewed, and basic analytics. See “Cookies” below.

How We Use Your Information

Your information allows us to:

  • Confirm and manage appointments
  • Provide clinical assessment, treatment planning, and ongoing care
  • Personalise your treatment based on your history and goals
  • Process payment and produce superbills for possible insurance reimbursement
  • Comply with legal and regulatory record-keeping obligations
  • Send you appointment reminders and (only if you opt in) clinic updates or offers

Sharing Your Information

We do not sell, rent, or trade your personal information. We share information only where:

  • Within your care team, when more than one therapist at National Lymphatic Centers is involved in your treatment plan.
  • With service providers, our booking and practice-management platform, payment processors, and secure email and SMS delivery providers, all under written confidentiality and data-protection agreements (including business associate agreements where HIPAA requires them).
  • When you direct us, for example, to send reports to your physician, insurer, or another healthcare provider.
  • When required by law, to comply with legal process, applicable licensing boards or regulatory authorities, or to prevent serious risk of harm.

Email and SMS Communications

If you opt in to marketing emails or SMS through one of our forms, online booking, or the chat widget on this site, you will receive periodic updates about treatments, events, and promotions, as well as appointment confirmations and post-visit care messages.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All categories listed above exclude text-messaging originator opt-in data and consent; this information will not be shared with any third parties.

By providing your mobile phone number to National Lymphatic Centers, through a form, booking platform, chat widget, or by texting us, you consent to receive recurring SMS messages from us. Consent is not a condition of purchase.

  • Message frequency varies based on your interaction with our practice (appointment confirmations, reminders, post-visit care, and, where you have opted in, promotional content).
  • Message and data rates may apply per your mobile carrier’s plan.
  • Reply STOP at any time to opt out of further SMS messages. You will receive one final confirmation, after which no further messages will be sent.

  • Carriers (AT&T, T-Mobile, Verizon, US Cellular, and others) are not liable for delayed or undelivered messages.
  • You can unsubscribe from marketing emails at any time using the link at the bottom of any email.

Data Security

We take reasonable physical, administrative, and technical steps to protect your information, including encrypted storage, access controls, secure clinic premises, and trusted service providers. Despite our efforts, no method of transmission or storage is 100% secure, and we cannot guarantee absolute security.

Retention

Health records are retained for the period required by applicable federal and Illinois state law. After that period, records are securely destroyed unless a legal retention obligation applies.
